This offer is limited to adult patients who receive a treatment with Restylane®, Restylane-L®, Restylane® Silk, Restylane® Lyft, Restylane® Refyne, Restylane® Defyne, Restylane® Kysse, and/or Restylane® Contour (a “Restylane Product”) in accordance with the products’ FDA-approved indications and labeling. You must be a member of the ASPIRE Galderma Rewards program. This is a limited quantity offer subject to availability from Galderma and your participating healthcare provider. To participate in this offer, you must receive (i) a treatment certificate coupon code from ASPIRE on or after September 1, 2021 and (ii) a qualifying treatment with one (1) or more Restylane Products in a single office visit and within sixty (60) days of receipt of the coupon code. Your healthcare provider may submit a valid $100 coupon code (for treatment with two (2) syringes of Restylane Contour) OR a valid $150 coupon code (for treatment with two (2) syringes of Restylane Contour and one (1) syringe of any other Restylane Product) to the ASPIRE program on your behalf at the time of treatment. Coupon codes shall expire sixty (60) days from the date issued.
You are eligible for this offer only if you pay for your entire treatment yourself and if no part of your treatment is covered by insurance or any other third-party payor. This offer excludes any treatment using a Galderma product that is reimbursed by Medicaid, Medicare, or any other federal or state benefit programs, including state medical assistance programs. You are not eligible for this offer if your private insurance, HMO, or any other health benefit program pays for all or part of their treatment. If any form of reimbursement is sought from a third-party, you may be required to disclose the value of the coupon code to that party. This offer is not available to patients who are Galderma employees or their spouses.
This offer is non-transferrable and available and valid only from participating healthcare providers. All treatment certificate coupon codes must be submitted by the treating provider via www.aspirehcp.com. Galderma will reimburse the provider for the face value of used coupon code, provided that the provider and patient have complied with these terms and conditions. Individual coupon codes are valid for one-time use. A patient may receive this offer only one (1) time. Promotion coupon codes cannot be combined with coupon codes from other ASPIRE consumer offers or promotions. Coupon codes have no cash value without simultaneous purchase of a treatment with one or more Restylane Products, and are void if sold, transferred or exchanged. Offer valid only in the U.S. and Puerto Rico (excluding all other U.S. territories), and void where prohibited, taxed or otherwise restricted by law.
