The human brain sees beauty in balance, natural proportions, and symmetry. The brain sees these things as a natural sign of health, vitality, and youth. Natural facial ratios are often subconsciously perceived as beautiful, harmony, and proportion signal balance to the eye—even when we don’t consciously realize it.

A great example is lip proportion. While the ideal shape varies by ethnicity, gender, and individual facial structure, a commonly referenced aesthetic guideline is a 1:1.6 ratio between the upper and lower lips, though 1:1 may be appropriate for some patients based on their ethnicity, age, or gender. In many naturally balanced faces, the lower lip is slightly fuller—about two-thirds of the total lip volume—while the upper lip accounts for about one-third. The lower lip also tends to project slightly more centrally, creating a soft, natural pout. Because of the variables involved, we customize each treatment to every individual.

When ratios are off, or mismatched, or not followed at all, the brain sees it as a distraction. I always tell patients the brain will “glitch” because it can’t make sense of what it is seeing. The brain perceives unnatural filler results through the “uncanny valley” effect, experiencing cognitive dissonance because the face appears almost human but is “not quite right” often appearing stiff, distorted, or overly puffy. The brain detects inconsistencies in facial animation, movement, and natural contours.

In our Practice, we all follow anatomical guidelines when injecting fillers into the human face, resulting in patients looking fresh, rested, and healthy, yet others can’t quite figure out what they’ve had done. Gender and ethnicity are never overlooked — they are fundamental to how we approach every treatment. The human face is not “one size fits all,” and injectable artistry should never follow a single template. There is no universal way to inject cheeks. There is no single method for lips. Injectors who practice with a fixed formula, rather than an individualized approach, risk creating results that look artificial or unbalanced, i.e., the “uncanny valley” effect.

If you ever look at one half of your face mirrored onto the other through an app, you will look entirely different based on which half you mirror.

The key is understanding anatomy. Filler must be placed where true volume loss has occurred. As we age, changes occur in our fat pads, bone structure, and soft tissue support structures that were established before birth. When we restore volume in alignment with this natural anatomy, the results look balanced and effortless. When filler is placed outside of those anatomical foundations, it can appear obviously unnatural. Thoughtful, meticulous placement in the appropriate facial plane, respect for structure, and a conservative approach are what keep results refined, harmonious, and beautifully undetectable.
